There are multiple levels of Sam protecting Dean with this scene. He pins Alastair to the wall to keep him from killing everyone, he “interrogates” him so Dean doesn’t have to anymore, and then, with a noticeable level of satisfaction, he kills the demon who personally tortured and broke his brother for 4 decades.

It’s okay, Dean, it’s gonna be okay. I’ve got him.
For Dean, this is it, their clock’s run out. Because Sam isn’t going to walk off this field, and therefore neither will he. He makes the choice, though there was never an option in his mind, to walk right in the middle of the armageddon’s ground zero. Because he won’t let Sam die alone. If Sam is going to die and he can’t stop it, he’ll die by his side. So even though his face is pummeled into oblivion and he just watched everyone else he cares about die, he calls out to Sam to let him know that it’s okay. Because he’s there with him. He won’t ever leave him, even now at the end of everything.
And for Sam, who has been kicking and screaming this whole time, doing everything his incredibly strong mind and soul can do to try to take back control and stop Lucifer, it’s literally the power of love that saves them. Saves everyone. The sun reflects off of the Impala and catches Lucifer’s eye for a moment, bringing into view the toy soldier Sam jammed into the ashtray as a child. And it opens a floodgate of memories for Sam, starting from that point and on, forever by his brother’s side. Memories of the past five years with Dean, riding shotgun and listening to the same 5 tapes over and over again. Laughter and tears, life-changing moments and little pleasures, hunting evil and being hunted by the police, prank wars and singing off-key, all leading up to the moment when his brother sold his soul for him and hugged him so tightly that it hurt.
Lucifer’s endless centuries of planning and stewing in the pit, as well as God-ordained prophecy and destiny, was completely undone by one thing that would seem so small in the grand picture of things. Something they overlooked, underestimated, didn’t even remotely care about.
Destiny itself was undone by the love between Sam and Dean Winchester.

Yet when Sam is hurt, even if it’s just a gash on his hand, Dean is tender and gentle, moving slow and careful as to not exacerbate things. But it’s not just the hand. Something is fundamentally wrong with Sam, it’s clear as day to him, though he has no idea how bad it really is yet. So he dotes on him more than he would have a few weeks ago, before the wall was broken. Like he did when Sam was young. And Sam watches Dean, a mixture of terror and confusion swimming in his eyes that are a heartbeat away from swelling up with tears as he struggles to keep his mind in reality. He looks to Dean like he did as he was growing up, searching for answers and subconsciously needing him to make it all okay.

At first, Sam goes in to shock, silently mouthing his brother’s name as the hairs stand up on his neck. It takes a moment and then this “Dean” talks, smiles and jokes like his brother would, and Sam’s blood goes cold with blinding rage. How fucking DARE this ghoul or demon or shifter or whatever the hell it is wear the face of his dead brother. He’s so fucking pissed and he surges forward to kill it, because seeing Dean’s face and it not actually being Dean is too much to handle. But it is Dean, and once that sinks in he can’t move fast enough to wrap his arms around him, something he’s dreamed about every night before his dreams went red and he lost him all over again. Later Dean accuses him of making a deal to rescue him, because Dean knows he did the same. He doesn’t doubt that Sam would do it for him, too. That’s the kind of relationship they have. Sam admits he tried, begged to sell his soul and how he’s so very sorry they wouldn’t take it. So damned sorry he was down there for so long and he couldn’t do anything. Then, with questions unanswered and a destination to head to, Sam slips the necklace out from under his shirt where it’s been safely kept close to his heart these four months, and gives it to Dean, just like he did 20 years ago.
